Foot Adduction / Abduction

Target−5° to +10° (slight out-toe is normal; in-toe is the larger flag)

Why measure at midstance

The long axis of the foot is most reliably visible when the foot is flat under the body — exactly the midstance frame. At initial contact the foot is still settling into ground contact (often rotating internally as it loads); at push-off it lifts at the heel and only the forefoot remains on the ground, which makes the heel landmark ambiguous. Midstance gives a clean, fully-grounded foot to trace from heel to second toe.

Why this matters

The foot's orientation at contact sets up the kinetic chain for the rest of stance. A neutral or slightly out-toed foot lets the toes load and push off through the big toe — the strongest lever on the foot. A markedly in-toed foot ("pigeon-toed") rotates the lever, so push-off goes through the lateral toes instead of the hallux, which is mechanically inefficient and is also associated with hip internal-rotation issues upstream.

Slight out-toeing (5–10°) is normal and biomechanically efficient — the line from the centre of the foot to the great toe sits closer to the line of progression that way. Aggressive out-toeing (>15°) is usually a habit pattern from walking biomechanics that has bled into running. It loads the medial arch and big-toe joint asymmetrically.

What good looks like

At contact the foot is pointed nearly along the direction of travel, with up to ~10° of out-toe. The pattern is symmetric L vs R. Through stance the foot rolls forward smoothly without significant rotation of the foot relative to the ground.

Common faults

  • In-toeing (>5° adduction) — foot points slightly inward at contact. Often paired with internally-rotated hips (femoral anteversion) upstream. Push-off shifts to the outer toes.
  • Excessive out-toeing (>15° abduction) — foot splayed wide. Common in walkers-turned-runners; loads the medial column of the foot.

Coaching cues

  • "Point your toes where you want to go." Visual cue, often paired with a chalk line or floor markings.
  • Address upstream rotation: hip-internal-rotation strength + mobility for in-toeing; hip-external-rotation strength for excessive out-toeing.

Related measurements

  • Pronation — in-toed feet over-pronate more on average; out-toed feet supinate more.
  • Knee Adduction — in-toeing rotates the tibia inward, which loads the patellofemoral joint similarly to dynamic valgus.
